What criteria define Systemic Inflammatory Response Syndrome (SIRS)? (6 things)
A
- Temp over 38.3C or under 36C
- Pulse over 90bpm
- RR over 20/min
- New confusion / drowsiness
- WBC over 12 or under 4x10^9/L
- Blood glucose over 7.7 mmol/L (non-diabetic patients)
2+ of the above present = SIRS

What is the Septic Six Bundle?
A
6 things that have to be done in the first hour of identifying Sepsis:
- Give O2
- Measure Lactate (Treatment performance marker)
- Take blood cultures
- Give antibiotics immediately (broad spectrum)
- Give fluid challenge of 30ml/kg for BP less than 90
- Measure strict fluid balance (consider catheterisation… to be able to measure input vs output accurately)

What are the SOFA criteria?
A
- Respiratory - PaO2/FiO2 ratio
- Haematology - Platelet count
- Liver - Serum bilirubin
- Renal - Serum creatinine (or urine output)
- Brain - Glasgow Coma Scale
- CVS - Hypotension and Vasopressor requirement
Suspected infection AND change in 2+ of above from baseline

How do micro-organisms trigger the inflammatory cascade? (4 things)
A
- Gram -ve (e.g E coli) bacteria main cause of sepsis
- Gram -ve have lipopolysaccharide (LPS)
- LPS activates MAC n NK cells
- MAC n NK activate +ve feedback loop —> amplifies immune response —> innate system responds quickly to infection
