202: Stroke Flashcards
typical stroke symptoms:
contralateral hemiparesis and dysphagia without loss of consciousness. sudden onset, related arterial anatomy,
MCA ischaemia symptoms
language and motor
Thalamus symptoms
hemianaethesia ( loss of sensation on one side)
occipital lobe symptoms
vision (hemianopia)
lacunar infarct is…
blockage of penetrating arteries supplying the basal ganglia, thalamus, internal capsule , pons. can see lacunae (cavity)
describe intracerebral haemorrhage
caused by hypertension, tumour, angiomas, amyloid angiopathy ( bleeding from small arterioles occurs)
Describe subarachnoid haemorrhage
caused by rupture of saccular aneurism ( outside the brain but inside the skull) or arterio-venous malformation. thunderclap headache and meningism and sudden loss of consciousness
describe cerebral venous sinus thrombosis haemorrhagic stroke
younger women on OCP. sub acute/evolving, secondary bleeding raised ICP and no arterial regional pattern. CT shows opaque sections (blood)
brainstem and spinal stroke symptoms
ataxia, diplopia, dysarthria, hemiparesis
right pons stroke symptoms
right cranial nerve 7 ( facial) and left hemiparesis
lower mid brain stroke symptoms
right cranial nerve 3 ( ocularmotor) and right hemiparesis
spinal vascular deficit symptoms
myelopathy
brainstem issue symptoms
opposite limb to lesion affected, same side of face though.
Classification: Total Anterior Circulation Stroke
major problems in carotid area leading to middle and anterior cerebral artery thrombi
Classification: Partial Anterior Circulation stroke
just middle or anterior artery
Classification: Posterior Circulation stroke
Vertebral or basilar distribution.
Treatment for thrombosis
IV rTPA ( recombinant Tissue Plasmnogen Activator)
List some long term anti platelet therapies:
asprin, dypridiamole, clopidogrel,
Long term anti clotting therapy
Warfarin
Brain zone borders susceptible to hypo perfusion
left frontal lobe and temporal horn of right ventricle
