Neurotrophins in dentin matrix proteins could play a significant role as signaling molecules for the innervation of the pulp-dentin complex during formation, repair, and regeneration. Neurotrophin 4/5 found in highest amounts.

3D endo software and CBCT resulted in significantly less stress, better working length determination, and less voids.

After surgery healing occurred with little to no regression of the buccal cortical plate without the use of grafting materials.
**Von Arx - “A 10 year follow up study of 119 teeth treated with apical surgery and root end filling with MTA”
Teeth treated with apical surgery and MTA root end filling had 81.5% healing. Many of the lost teeth were extracted due to longitudinal root fractures.

Greater healing in high concentration (5%) than low concentration (1%) but no SSD.

30% had pulpal complications. 65% irreversible w/in 1.2 years. 35% necrotic after two years. 8.7X more pulpal complications without crown. 71% of teeth with reversible pulpitis when crowned remained healthy for over 3 years.

Avg. deviation for guided 1.7mm, 2.6mm for free hand. 11/24 success free hand, 24/24 success guided.

CBCT and micro-CT had god accuracy and reliability, first file to bind was inaccurate and unreliable. Mean canal diameter for CBCT was 0.22mm and for first file to bind was 0.15mm.

Resilon had significantly greater rate of degradation than GP.

1st molars wider M-D at CEJ. Thinner dentin on palatal aspect of root in middle/coronal 1/3. Minimum 40 master apical size. 76% no apical constriction. 3.5mm root end resection.

Multiple instruments, high volume of irrigation, and long retention time was best.
**Nixdorf - “Accuracy and reliability of root crack and fracture detection in teeth using MRI”
CBCT and MRI had similar sensitivity (true +) (0.59) and specificity (true -) (0.83 MRI and 0.9 CBCT)
Siqueira - “Matching dimensions of current available instruments with the apical diameters of mandibular molar mesial root canals obtained by micro-CT”
78% of apical canals not completely by any instrument, ideal instrument size/taper to include all canal walls in apical prep is too large to be safe for clinical use.

Overall success of 93% on PA vs 85% on CBCT with no difference between MTA or root repair material.

Application of 20% alpha-tocopherol not effective in counteracting deleterious effect of bleaching on bond strength, need to wait about 2 weeks before restoring.

Sterile root canals released greater amounts of TGF-B1.

New hard tissue formation at the resection plane and within former apical defect in most cases at 5 years, reestablishment of the cortical plate lagged behind. Periapical healing is not complete after 1 year, need longer follow up.
Georgiou - “apical periodontitis is associated with elevated concentrations of inflammatory mediators in peripheral blood”
Apical periodontitis contributes to low grade systemic inflammation through increased C reactive protein, IL-6, and C3 levels
**Von Arx - “Proximity of vascular bone channel in the lateral sinus wall to root apices of maxillary first molars”
The vascular bone channel was found closer to the MB root (6.18mm) than DB root and mostly located intrasinusly.
