MC drug problem in the ED
1) marijuana, 2) cocaine, 3) street opioids
which group is more likely to use alcohol, tobacco, and other drugs
younger pts and males
1st thing to do for pt w/depressed central respiratory drive presenting to ED
ABCs
opiate and benzodiazepine intoxicataion: tx?
naloxone or flumazenil
drugs that cause: MIOSIS
sympatholytic agents, opiods, nicotine
drugs that cause: MYDRIASIS
sympathomimetic agents: LSD (lysergic acid diethylamide), cocaine, amphetamines and derivatives
drugs that cause: horizontal-gaze nystagmus
barbiturates, ethanol,
carbamazepine, phenytoin, scorpion envenomation
drugs that cause: horizontal-, vertical-gaze, and rotatory nystagmus
phencyclidine (PCP)
how does nicotine abuse present?
multiple nicotine patches, smokeless electronic cigarettes
who is mecamylamine not suitable for?
pts who are vomiting, convulsing, hypOTN
Purification of heroin
1) white, milky sap from the poppy seed oozes out –> turns black –> scrape it and wrap it
2) mixed w/lime in boiling water, take the top morphine layer, reheat w/ammonia –> brown again
3) dry in sun
4) ether and HCl purification = heroin
