psychosis
mental disorder w/impairment of thoughts, reality, ability to communicate interfering w/capacity to deal with reality
classic characteristics of psychosis
impaired reality testing, hallucinations, delusions, and illusions
key features of schizophrenia spectrum disorders: POSITIVE (not typically seen in normal behavior)
delusions, hallucinations, disorganized thinking (speech), grossly disorganized or abnormal motor behavior (including catatonia)
key features of schizophrenia spectrum disorders: NEGATIVE (disruptions of normal emotions and behavior, deficits)
diminished emotional expression, avolition (i.e. decrease in motivated and self-initiated purposeful activity), alogia (diminished speech output), anhedonia (diminished ability to experience pleasure), asociality (lack of interest in social interactions)
*typically need help with everyday tasks, hygiene problems, seen as lazy
dx of schizophrenia
pt must have been sick for 6 months or more
active sx lasting at least 1 month
schizophreniform disorder
first episode, less than 6 months duration
schizoaffective disorder
major mood disorder concurrent with schizophrenia-like psychosis
-mood sx last for 2 wks + psychosis
delusional disorder
circumscribed and often BIZARRE delusion, in an otherwise normal-appearing pt
brief psychotic disorder
transient psychosis, precipitated by stress
classification of schizotypal disorder
personality disorder (cluster A) + schizophrenia
psychotic disorders common in medical setting
substance/medication induced psychotic disorder
psychotic disorder due to general medical condition
catatonia
delirium
dementia
substance/medication-induced psychotic disorder
persistent, prominent delusions and/or hallucinations that developed during or soon after substance intoxication or withdrawal, or after Rx that can produce these sx (NOT the same thing as delirium)
*the condition would not be better explained by a pre-existing psychotic disorder
psychotic disorder due to general medical condition
evidence from hx, PE, labs that psychosis is due to the physiological effects of a med condition, did not precede the onset of the medical condition, does not exclusively occur during delirium
examples of psychotic disorder due to general medical condition
malignant lung neoplasm, MS, brain injury, hypothyroidism
may improve w/improvement in the medical condition
catatonia
severe psychomotor disturbance, may be associated w/schizophrenia, depressive stupor, etc.
**can be life-threatening
