Mechanism of action
Inhibits two enzymes involved in formation of activated vitamin K, inhibits synthesis of new clotting factors
indications
Prophylaxis arterial thomboembolism, DVT, pulmonary embolism, long-term indwelling catheters
precautions
dental procedures, CHF
contraindications
recent trauma, active internal bleeding, serious bleeding disorders, intracranial hemmorrhage, severe Hypertension, bacterial endocarditis, hepatic and renal impiarment, pregnancy

heath Hx; moniter PT/INR; moniter urine, stool, liver, blood; moniter risk groups for non-adherence
teaching
avoid accident prone activities, report bleeding, stop and report hepatitis, use consistently, avoid high-fat diet, avoid excessive alcohol, inform dentist, avoid pregnancy, avoid changes in levels of vitamin K foods
