Mechanism of action
activates the enzyme antithrombin III, which inhibits thrombin
indications
Acute thromboembolic disorders, DVT, Pulmonary embolism, Unstable angina, Evolving MI, Prophylaxis
precautions
Peripartum patients, renal or hepatic impairment
contraindications
Active internal bleeding, Serious bleeding pathologies, severe HTN, Recent trauma, Intracranial hemorrhage, Bacterial endocarditis, Sulfite sensitivity, Severe thrombocytomenia
drug interactions
Potentiated effect with oral anticoagulants; Bleeding with drugs that inhibit platelet aggregation; Inhibited anticoagulation with nicotine, digoxin, tetracyclines, antihistamines; Ginger, garlic, green tea, feverfew, and ginkgo
pregnancy category
C
adverse effects
Abnormal bleeding, Hemolytic induced thrombocytopenea, mild allergic symptoms, osteoporosis, skin lesions
nursing considerations
Complete health history, Baseline blood tests, Monitor aPTT, Collect blood for testing 30 minutes prior to dose, Monitor for signs of bleeding, Apply firm pressure _ 10 minutes for needle sticks, Reduce risk of trauma, Keep heparin antidote readily available
Teaching
avoid accident prone activities, report bleeding problems immediately, menstruation may increase, avoid smoking and alcohol, do not take other drugs or supplements, learn subQ injection technique, inform dentist.
