To provide guidance on matters required for a workplace inspection regime for Cook Colliery, in accordance with Regulation s309.2. Inspections are not to be considered as a primary control for hazards, they are conducted to monitor the mine environment and any associated hazards. Inspections are used to maintain an acceptable level of risk at the workplace by confirming the effectiveness of current controls.
What is the procedure for closing the district ?
Ensure all persons are removed from the district,
isolate power to the district,
turn off water at the pipe range
cross stick and tape off with no road tape all travelling ways outbye of the district.
Then indicate on the notice board the reason for closing the district
change the sign to CLOSED.
Carry out a belt shutdown inspection.
Finally ring control to notify him of the closure.

List 11 Items
1)The presence of flammable gases or contaminants in the atmosphere.
2)The adequacy of the following –
Ventilation;
Coal dust inertisation;
Emergency, first aid and fire fighting equipment.
3)The condition of the following –
Ventilation control devices;
Auxiliary fans;
Surfaces over which persons may travel or vehicles may be driven;
The support for the roof and sides of the workings.
4) The stability of roadways in the workings.
5) Indications of heating or fire.
6) Abnormal water inflow.
7) Plant malfunction.
8) The proper functioning of communication and monitoring systems.
9) Excessive accumulation of mud, water or coal.
10) Thermal environmental conditions
11) Check state and condition of electrical installations and cabling.

Select, test and calibrate gas testing instruments
Read analyze and record the barometer readings
Read Analyze and record the quantities and pressures of the main operating fans
Read analyze and record the wet and dry bulb temperatures
Check gas monitoring system
Check communication system
Read and countersign previous inspection Statutory reports relevant to the zones applicable
Advise Surface Controller of planned route and destination
Check PPE
List 2 things you would carryout to prepare for a pre shift inspection on the surface before going underground? extra to the items detailed in the above question?
For Pre shift inspection
Take 2 Cylume sticks and or an additional cap lamp .
Co-ordinate a communication schedule/protocol with the surface controller
List 10 items to be checked on a Conveyor belt inspection?
Appropriate clearances are maintained
Belt conveyor alignment Tension and Clearances are being maintained.
Accumulations of spillage of coal, coal dust and lubricant is being prevented.
Belt conveyor structure, idler and return rollers are in good working order and running freely.
There is no overheating of driveheads, idlers and return rollers or any parts of the conveyor.
Belt conveyor including joints is in good condition.
The effectiveness of the remote control and signaling system is being maintained.
The presence of combustible material other than leveling wedges is prevented.
Belt scrapers are in good order.
Fire fighting equipment is in good order and in position. Including fire hydrant fire depot and signage
All guards are in position.
All lighting is in place at the transfer point
Are skirts in place at the feeder discharge
Is the area around the feeder including car approaches generally clean
For belts becoming non-operational for a period greater than ?? hours, a full belt inspection as described above shall be performed as soon as practicable after shutdown. What is this time?
2 hours
Where a belt is started and run for a period not greater than ?? minutes, a shutdown inspection shall not be required
What is this time?
30 Minutes
What is recorded Immediately at the end of the inspection enter the results and any other relevant information on to the Zone inspection board located at the interface zone of that district being inspected?
The information shall state
Date and time inspection commenced
Date and time inspection ceases to be effective
The condition or status of the panel or district
A legible signature of the inspection officer
A written duplicate report on the inspection findings shall be completed in the relevant format (outbye/Productionl) report form
Any unsolved safety issues are to be immediately bought to the Shift Supervisors attention, who then shall notify the Mine Manager.
Where are deputies site inspection boards at cook
These are located at Transformers sites and SCSR Cache locations and shall be signed and dated for that particular inspection.
When are these areas inspected?
Any part of a mine ventilated by an auxiliary fan which has failed
Before persons other than those inspecting re-enter that part and before electrical power is restored to the auxiliary fan for that part
When are these areas inspected?
NERZ Main traveling roads portals to ERZ interface
Once each shift
When are these areas inspected?
NERZ Conveyor belts operating continuously for in excess of 2 hours
Once per shift
When are these areas inspected?
Homotropal conveyor belt roadways where the conveyor is operating
Once each shift
When are these areas inspected?
ERZ1 Where coal or stone is being mined or (Primary) strata control activities are being carried out
Continual inspections. An ERZ Controller must be present in his inspection district while these activities are being carried out
When are these areas inspected?
ERZ1 Development panel Where no production or strata control activities are being carried, where an Auxiliary fan is not operating but other activities are being carried out
