Incident 11.1.1.1
Occurrence associated with the operation of an Ac which affects or could affect the safety of operation.
Serious incident 11.1.1.2
Incident involving circumstances indicating that there was a high probability of an accident associated with the operation of an Ac which takes place between person on board and off board.
Accident 11.1.1.3
- a person is fatally or seriously injured
- ac sustains damage or structural failure
- ac is missing or is complete inaccessible.
Serious injury 11.1.1.6
An injury which is sustained by a person in an accident and which:
- requires hospitalization for more than 48hrs (within 7 days of injury)
- results in a fracture of any bone
- involves injury to any internal organs
- involves second or third degree burns
- involves exposure to infectious substances or injurious radiation.
Fatal injury 11.1.1.7
Injury which is sustained by a person in an accident which results in his or her death within 30 days of the date of the accident.
DG incident vs accident 11.1.1.4/5
Accident = fatal or serious injury to a person or major property damage related to DG.
Incident = dg resulting in injury to a person, property or environmental damage, fire, breakage, spillage etc.
Occurence reporting by Cpt 11.1.2.1
- ensure dispatch is informed
- write ASR
- entry AML (damage Ac)
- ASRs must be submitted to SPL/SC within 72 hrs!!
Cockpit voice recorded / flight data recorded 11.1.3.3
- not disabled or switched off during flight
- serious incident / accident: not intentionally erased, deactivated immediately after flight.
After incident CVR 11.1.3.3
Pull and clip CB of CVR, make entry in AML, clearly marked as INCIDENT.
Air traffic incident 11.1.4.1
- notify ATC about incident
- inform your write ASR (near collision, failure ATC facilities, faulty procedures or lack of compliance with procedures).
Deaths 11.1.6.1
Cpt must fill out “preliminary certificate of death” - within 24 hrs.
- deceased should be laid down, hands crossed, eyes closed (out if sight other pax)
- deceased must be offloaded first
- transferring to authorities; cpt shall hand over short written statement
- cpt looks after belongings
Births 11.1.6.2
Cpt must fill out “ preliminary certificate of Birth” - within 24 hrs.
VSR 11.1.3.2
Voluntary safety report - to collect and submit details of occurrences and safety related information.
Reasons for an ASR
- incident/accident
- TCAS RA
- air traffic incident
- bird strike
- DG incident
- unsafe situations
- SCD etc.
Different reports to be used to inform company:
- ASR
- trip report
- VSR
- OSR

OSR 11.1.5.2
Occupational safety report - when a crew member encounters an unsafe situation, incident or accident involving the personal safety of the crew member.
Trip report 11.1.5.3
- legal aspect as required by law other than accidents and incidents
- deviation from operational procedures not resulting in accident / incident
- factual observations, which may result in cost effectiveness and efficiency of the KLM operation.
ASR 11.1.5.1
Air safety report - to report all safety related events during the operation of flight.
