07 - mediastinal and pleural Flashcards
1
Q
(mediastinal shift of the cardiac silhouette)
1-2. what two causes?
A
- volume loss of lung (atelectasis): heart shifts towards affected side
- vol increase cause by intrathoaracic or pulmonary mass -> heart shifts away from affected side
2
Q
(mediatstinal masses)
- most common cause?
A
- lymph node enlargement (2° to lymphoma, neoplasia, fungal dz)
3
Q
A
4
Q
(pleural effusion)
1-3. what three main things are seen?
- possible fluids?
- what are 5 causes?
A
- ^ thoracic opacity
- pleural fissure lines between lung lobes
- separation of lung from thoracic wall with fluid opacity in the pleural space
- transudates, modified-transudates, exudates
- right sided heart failure, chylous effusion, pyothorax, hemorrhage, neoplastic effusion
5
Q
(pneumothorax)
- air in the pleural space -> results in what?
- if lung lobes collapsed what is this called?
A
- separation of lungs from thoracic wall, heart dorsally elevated
- tension pneumothorax (emergency!!!)
6
Q
(thoracic wall)
- masses may be neoplastic or infectious, hemorrhage secondary to trauma is generally more diffuse
- rib fractures commonly overlooked
