02. Clinical Reasoning & Questions Flashcards
hours spent by PA student on H&P
Year 1 - 4.5 hr; Last year - 2 hr
hours spent by seasoned provider on H&P
Resisdent - 0.5 hr; Clinician - 0.25 hr
5 styles of clinical reasoning
Differential diagnosis model; Hypotheticodeductive model; Exhaustive model; Algorithmic model; Heuristic model
Differential diagnosis model
Process of considering the possible causes of the patient’s complaint before making the diagnosis; Cyclic process; Testing the hypothesis results in diagnosis
Hypotheticodeductive model
Generate hypothesis; Gather data for hypothesis testing; Test hypothesis; Is one hypothesis valid?; Take action
Exhaustive model
Possibilities approach; Comprehensive H&P; All diagnostic possibilities considered; Data are carefully sifted
Algorithmic model
Systematic branching decision tree (pathways); Objective data are used to choose pathways; best with discrete data
Heuristic model
Discovery through pattern recognition/syndromes; Informal methods or experience; Example = Reactive arthritis
Define “rare disease”
Any disease or condition that affects less than 200,000 people in the US
Exposure
Variable (possibly) associated with a disease state
Latent period
Period after exposure when disease is not present
Early symptoms
Symptoms occuring between latent period and overt/disease onset
Disease onset
Beginning of overt disease state
Protocol
Code prescribing strict adherence to correct etiquette & precedence
Guidelines
Official recommendation indicating how something should be done or what sort of action should be taken
