What is Rickettsia?
Gram-negative rod, obligate intracellular bacteria. Vector borne.
What is classic presentation of Rickettsia?
fever-headache-rash
Where do rickettsia replicate?
in cytoplasm of endothelial and muscle cells of small blood vessels
What disease does R. rickettsia cause? What are its presentations?
Rocky Mountain Spotted Fever. Flu symptoms withere severe headache and a rash that spreads from ankles and wrists to soles, palms and trunk. Muscle pain (especially calf)
How is RMSF diagnosed?
Weil-Felix test and confirmed by convalescent sera.
What does Weil-Felix test for?
measures cross-reactivity of anti-rickettsial antibodies to Proteus OX strains.
What is treatment for RMSF?
doxy
What disease does R. prowazekii cause?
epidemic typhus. Rash spreads from trunk to extremities (opposite of RMSF). Flying squirrels?
How is R. prowazekki transmitted?
human-to-human via body lice (not a zoonosis)
How is erhlichia transmitted?
by ticks
How does erhlichia replicate?
intracellular in white blood cells.
E. chaffeensis - monocytes
A. phagocytophilia - granulocytes
Would a Weil-Felix reaction be positive or negative for Ehrlichia?
negative
How do you diagnose E. chaffeensis?
Wright/Giemsa stain of blood smear. Look for morulae in MONOCYTES
What causes Rocky Mtn. SpotLESS fever?
A. phagocytophilia.
How is A. phagocytophilia infection diagnosed?
wright/giemsa stain of blood smear. morulae in GRANULOCYTES
