Phalen’s Test
Median nerve compression
reverse phalen’s (prayer) - also tests median nerve under differs stress
Finkelstein’s
DeQuervain
stenosing of extensors pollicis brevis and abduction pollicis longus tendon
Digital Allen’s test
have pt squeeze hand into wrist 5 times, keep tightly closed, cut off artery flow, open hand, release arteries, look for return of color
axial load tests of fingers an wrist
test for fracture, apply axial load through long bones
carpal compression test
looking for re-production of symptoms/ pain
push on the anterior wrist crease where the median nerve runs through the carpal tunnel
indicative of carpal tunnel syndrome
Wrist glide testing
ulnar glide, dorsal glide, palmar glide
(+): increase in pain, change compared with opposite side
indicates: ligament sprain, trauma to TFCC,
decreased glides: adhesions or stiffness
inter carpal joint play: same as above
grind test
performed on the 1st CMC (thumb)
look for pain, popping, clicking at joint - possible fracture, sprain, etc
Grip Dynamometry
injured non-dom hand:: more than 10% bilateral strength difference
injured dom hand: more than 5% bilateral strength diff
indicates: weakness - must determine underlying cause
Murphy’s sign
if 4th knuckle is dropped - indicates lunate dislocation
Jersey finger
can’t flex the DIP, make a fist look for unflexed finger
flexor tendon injury
Mallet finger
can’t extend DIP
extensor tendon injury
TInel’s sign
tap radial, median, nerves to look for reproduction of signs and symptoms
