Delayed PI - left open to allow infection to subside before closing

Factors affecting wound care

What to look out for on a wound
Pain Exudate Wound bed - infected red it Infection Size Depth Is there a fistula / sinus formation
Wound tissue types
Granulation tissue - red healing tissue
Epithelial donor tissue - like donor kebab, thin, fragile, needs steristrips
Slough - dead white blood cells that need to be removed - comes off with dressing, yellow, the surrounding tissue is not red suggesting no infection
Necrotic tissue - needs to be
Infected - angry, surrounding tissue needs to be cleaned
Name a type of biological debridgement
Maggots - leave them for three days under the wound to eat away the infection
What to ask when dressing the wound
What caused the wound
We are going to cover it
Allergic to anything
Pain - prescribe paracetamol
Any bleeding disorders
Tetanus injection - if not then we’ll make sure you get your jab
Process
Check rate of saline
Ask pt to lift arm to put paper underneath
Pass a wet swab from clean hand to dirty hand then swipe through ONCE each
Messages to go
Don’t get it wet
If you do, here are some dressings
Come back if painful, fever or unwell
Make an apt 7 days later with GP
