Normal Menses
-every 28 days +/- 7 days
-21-33 day cycle
-avg duration 4 days
-no more than 7 days
> 7 days is abnormal
Avg blood loss:
-35 to 50 cc per cycle
-95% women not heavy bleeders
- >80 cc or more is abnormal
—-hard to evaluate with different devices
*ask what type of fem products using
Menorrhagia
-Prolonged bleeding at regular frequency
- > 7 days or > 80 cc but pt has reg freq
-very common
- up to 14% of women who are healthy otherwise
Metrorrhagia
-breakthrough bleeding between periods
-May be accompanied by:
*Mittelschmerz
-pain that occurs w/ ovulation
OR
*Anovulation
-PCOS and lining sheds irregularly
Menometrorrhagia
Prolonged uterine bleeding that is occuring at irregular intervals
- spots every day or bleed q3-4 days
-bleeding w/ intercourse or heavy exercise
May not be ovulating thats causing irregularity
Oligomenorrhea
reduction in freq of menses
- not menstrating every month in a predictable pattern
Amenorrhea
-no menses for at least 3 to 6 months
Primary: never had a menses ever
-structural problem (imperforate hymen, missing ovaries,etc.)
Secondary: no menses for 3-6 months after reg cycle has been established
*cant have secondary in 1st 2 years of menarch; very common to be irregular 1st couple years and not established
Hypomenorrhea
reduction on number of days
- usually s/e of medication (BC)
Menarche
age of first menstruation
-Avg 12 yrs but getting younger
Menopause
absence of cycles for 12 months
-avg 51 yr
*Menstrual bleeding stops:
- NSAIDS decrease prostaglandin/bleeding
——prost. can cause contractions and expulsion bleeding
-Endometrial healing and cessation of bleeding also occur when estrogen levels rise
