What is the Transtheoretical Model and Stages of Change?
A theory that helps clients make progress of behavior change through six stages. Precontemplation, contemplation, preparation, action, maintenance and termination
What is precontemplation?
The stage where the client says “I can’t” or “I won’t”, no intent to change in the next six months, no motivation or reason to change. Example: I know I should quit drinking so much caffeine!
What are precontemplation goals?
Validate client’s lack of readiness. “The decision is yours, this is your life.
Encourage self exploration, don’t try to push action. Don’t sound judgmental in tone of voice.
What is contemplation?
The stage where the client says “I may”. Client states his or her intent to change in the next six months but may be not quite there due to personal reasons. Client is sitting on the “fence”. Example: My sleeping schedule is all messed up, I can’t keep this up, I’m tired all the time but I can’t quit this week.
What are contemplation goals?
Validate that client is not ready to make a change, clarify that it is the person’s decision, decisional balance, explore the pros and cons of change and staying the same, research resources and journal
What is preparation?
The stage where the person says “I will”. The client plans to take a step towards change in the next month. Offers a window of opportunity and test the waters. Example: I bought some decaf tea to drink instead of coffee.
What are preparation goals?
Problem solve a plan, brainstorm ideas with the client, identify obstacles and ideas that work best for the client. Encourage the client to further explore ideas and small steps. Encourage support system.
What is action?
Client has taken active steps to change
behavior for fewer than 6 months:
Example: “For the last few weeks, I have been trading out one cup of coffee for one cup of my
decaf tea during my night shift.
What are goals for the action stage?
Further explore client’s support system
Provide affirmations for positive behavior changes and small successes Continue brainstorming the plan

Example: “I have been caffeine free since January and I love my decaf peppermint tea! Maintenance Goals
