What is the six step planning model for community health planning
Manage the planning process, conduct a situational assessment, set goals, audiences, and outcome objectives, choose strategies and activities and assign resources, develop indicators, review plan
What does SWOT stand for and what is it used for
Strengths, Weaknesses, opportunities, threats
Used as a planning and evaluation framework
What is the logic model?
Helps expose gaps and show an overview of programs with appealing visual clarity
Inputs, outputs, short and long term goals
What is an example of an input in the logic model?
People/groups that can assist in this project: PH, family agencies, midwifes, doctors, IHA, schools, daycares, hospitals, SLP, the city
What are outputs in the logic model
Indicate that a program is being implemented as planned (basically you made/arranged what you set out to)
Ex. Brochure, workshop, family fair
What are activities in the logic model
Stuff you can do to interact with the community and raise awareness
Ex. Campaign, events, education
What is the audience in the logic model?
People who you are trying to reach
Ex. Parents, preschool, daycare association
What are short term outcomes in the logic model
Goals you want to achieve pretty quickly that aren’t too big
Ex. Increased interaction with PH, participation at events, increased inquiries
What are Long term outcomes in the logic model
Big overarching goals that the short term outcomes will likely work towards
Ex. Increased immunization in preschool
What are the steps of the planning-implementation-evaluation cycle
Analyze situation, identify problem, consider possible alternatives, select best alternative, design program, evaluate program, analyze findings, utilize results
What are the elements of implementation of community health programs?
Education, engineering, enactment, enforcement
What is social justice
Justice in the terms of the distribution of wealth, opportunity, and privilege within a society
Equity
Being fair and impartial, everybody gets what they need to meet their needs
How are social justice, equity, and population health linked?
A population health approach aims to improve the health of groups of people while reducing health inequalities and underlying determinants through multilevel policy change.
A nurse approaching the population from the prospective of social justice and equity would be focused on:
