Week 4: Reproductive Pharmacology: Drugs to prevent/treat postpartum hemorrhage Flashcards
Oxytocin MOA
Stimulates contraction of myometrium
Oxytocin clinical uses
Prophylactic administration during the third stage of labor for prevention of postpartum hemorrhage
Oxytocin side effects
headache
Oxytocin contraindications
risk of water intoxication (but unlikely with short administration schedule)
Contraindication: hypersensitivity
Oxytocin note
prophylactic use may reduce the need for therapeutic Uterotonics (eg methylergonovine, misoprostol)
Methylergonovine MOA
- Cause generalized smooth muscle contraction (increases uterine muscle tone)
Methylergonovine clinical uses
Treat postpartum hemorrhage
Methylergonovine side effects
- Nausea
- vomiting
Methylergonovine contraindications
- preeclampsia
- hypertension
- toxemia
- hypersensitivity
Carboprost (15-methyl PGF2α) MOA
- Increase uterine contractility and cause vasoconstriction
Carboprost (15-methyl PGF2α) clinical uses
Treat postpartum hemorrhage
Carboprost (15-methyl PGF2α) side effects
- Nausea
- vomiting
- diarrhea
- hypertension
- headache
- flushing
- pyrexia
Carboprost (15-methyl PGF2α) contraindications
caution in patients with asthma or hypertension
Contraindication: known hypersensitvity
Misoprostol (PGE1 analog) MOA
Increase uterine contractility and cause vasoconstriction
Misoprostol (PGE1 analog) clinical uses
Treat postpartum hemorrhage
