26
Q

What are some questions to ask women who are pregnant?

A

LPM?
How many weeks are you?
Prenatal cares?? (appts)
Have any previous pregnancies? (Grav #)
Any contractions?? (time them)
Complications in previous pregnancies?
Medical hx?
Complications with current pregnancy?
Any abnormal discharge?
Water broke?? Clear or discoloured? Odour?
Previous C-section?
Bloody show?
Presenting parts?
How many kids are at home?
Meds? Prenatal vitamins?
Positioning of baby?
Multiple babies in the womb?
Miscarriage?
Fetal movement?
Labour time last time?

27
Q

How do you time contractions?

A

frequency and duration

28
Q

What is frequency of contractions?

A

measured as in the interval between the time of one contraction and the start of the next

29
Q

What is and how do you time the duration of the contraction?

A

timed from when the contraction starts to when it ends

30
Q

What are Braxton Hicks contractions?

A

false labour pains

contractions of the uterus that typically are not felt until the second or third trimester of the pregnancy

body’s way of preparing for true labour but they do not indicate that labour has begun

31
Q

What is stage 1 of pregnancy?

A

the longest stage, contractions start and ends with 10cm dilated

32
Q

What are the two stages in STAGE ONE of labour?

A

early and active

33
Q

What do the uterine contraction that gradually increase in:

A

frequency
strength
length

34
Q

The fetus descends into _____ ?

A

Pelvis

35
Q

The cervix softens and…..

A

shortens
thins
dilates

36
Q

What is the max amount of dilation in cm?

A

10cm

37
Q

How does the pt present in the beginning of labour? (early labour)

A

walking and talking through contractions

38
Q

What are the frequency, length and strength of contractions during the early stages?

A

far apart and irregular

20-30 seconds long

mild

39
Q

During active labour…. what would pt presentation be?

A

difficulty walking, and talking through contractions
distressed, vomitting, shaking, sudden rupture of membrane (water broke), overwhelming emotions

40
Q

What are the frequency, length and strength of contractions during the active stages?

A

3-4 mins

45-60 secs

strong!!!

41
Q

What is stage 2 of labour?

A

starts when cervix is fully dilated
pt feels like PUSHING
end when baby is born

42
Q

What are the frequency, length and strength of contractions during the the second stages (when pt is fully dilated)?

A

<2mins

60-90 secs

STRONG

43
Q

From a clinical perspective… when does labour start?

A

when dilation BEGINS

44
Q

When does a paramedic LOOK?

A

near term

LOC is decreased /history is unavailable

inconclusive or indicates that labour was on going to prior to decrease in LOC

vaginal bleeding is HEAVY and the pt is hypotensive or in SHOCK

Anything HIGH RISK

45
Q

What is imminent birth as per the ALS?

A

crowning or other presenting parts are visible

46
Q

what is PRIMIPS?

A

presenting part is visible during and between contractions

maternal urge to push or bear down

contractions are less than 2 mins apart

47
Q

What is multips?

A

has had multiple kids!!

48
Q

What is stage 3 of labour???

A

from the birth of baby to the delivery of the placenta

usually occurs 5-30 mins after birth

49
Q

What would a paramedic observe in the third stage of labour?

A

lengthening of the cord
sudden gush or trickle of blood
uterine contraction

50
Q

What is the most dangerous stage of labour? and why?

A

placenta delivery

this is the highest risk stage because of increased risk of bleeding

51
Q

When the placenta is delivered what should we be looking for?

A

inspect for wholeness
place in a plastic bag from the OBS kit

label it with the mothers name and time of delivery

transport it with the maternal or neonatal pt

52
Q

Should the delivery of the placenta delay transport?

A

NOOOOOOOOO

53
Q

What are the steps of preparation in a normal delivery?

A

maintain pt privacy
prepare delivery station
open up the OB kit
update dispatch and ask for second crew
get pt on a firm surface
if they don’t want to move, deliver in any position
prepare neonatal resusisiton station
make sure environment is warm
have towels and blankets
use sterile gloves

54
Q

If a midwife is on the scene…

A

let em take over!!!!!

55
Q

What are the 7 steps of normal delivery?

A
  1. decent: fetus moves down toward the pelvis and becomes engaged]
  2. flexion: fetal chin to chest
  3. internal rotation: fetal occiput turns toward maternal pelvis
  4. extension: birth with head facing down
  5. restitution: baby turns head to the side
  6. external rotation: fetus turns to deliver the shoulder
  7. expulsion: birth of baby
56
Q

How far should you clamp the cord and WHEN?

A

when it stops pulsating up to 2 min mark

Clamp the umbilical cord in two places using the OBS clamps:

  • Approx.15 cm from the neonate’s abdomen and approx. 5-7 cm from the first clamp.
  • Cut the umbilical cord between the clamps using the OBS scissors.
