0-5 Scale based on wound depth and presence of infection

A
Primarily diabetic ulcers but can be used for neuropathic, ischemic, or arterial etiology ulcers

No open lesion, but may have pre-ulcerative lesions, healed ulcers, or presence of bony deformity
