W7_04 Coma Flashcards
define coma
deep unresponsiveness where the patient cannot be aroused to respond to stimuli
awareness results from what?
activity in the cerebral cortex
arousal results from what?
activity of brainstem and diencephalic structures
4 levels of arousal?
alert, obtunded, stupor, coma
arousal and awareness in coma?
none in either
arousal and awareness in vegetative state?
high arousal, but no awareness. can still laugh but unaware.
arousal and awareness in minimally conscious state?
high arousal but little awareness
arousal and awareness in locked in syndrome?
high in both.
what structures must be affected to cause a coma?
both cerebral hemispheres or the ascending arousal system (paramedian region of upper brainstem or diencephalon)
where is the lesion for locked in syndrome?
caudal pons (just below rostral pons, the area of arousal)
name some causes of coma
compressive lesions; destructive lesions; diffuse intrinsic (e.g. epilepsy, infection); metabolic brain disease; psychiatric causes
false localizing signs are produced at which anatomical location?
kernohan’s notch (left cerebral peduncle near the tentorial edge)
uncal herniation can cause what?
hemiparesis (cerebral peduncle); cortical blindness (PCA occlusion); eye palsy (3rd nerve); decreased arousal (brainstem)
left paramedian pontine lesion causes what?
1.5 syndrome
right lateral pontine lesion causes what?
gaze paralysis (can’t move towards side of lesion)
