How is vulvar cancer different to vulvar dysplasia?
Dysplasia refers to abnormally differentiated cells contained entirely within the epithelium - above the basal membrane. Violation of the BM indicates invasiveness and therefore cancer
What are the most common cancers of the vulva?
-epidermoid squamous
-melanoma
-undifferentiated
-sarcoma
Also Adenocarcinoma and Bartholin’s gland cancer (rare)
RFx vulvar cancer?
- Advanced age
- Hx dysplasia vulva
- Low SES
- Smoking
Sx vulvar cancer?
-Itch
-Palpable mass
Pain, bleeding, ulceration, dysuria also possible.

Lymphatics. Indolent growth followed by met
Most common first met site of vulvar cancer? Later spread?
Superficial inguinal nodes. Then
through fossa ovalis to deep femoral nodes then iliac chains.
