- in older patients, unifocal, from differentiated VIN, adjacent to LS and squamous hyperplasia

- punch biopsy

- direct extension

- haematogenous

Figo staging of vulval cancer
A
1: confined to vulva
2: extension to perineal structures with no nodes
3: positive inguino-femoral nodes
4: invades other regional or distant structures

Treatment of vulval cancer
A
Surgery: radical wide local excision or radical vulvectomy
- bilateral groin node dissection
Radiotherapy for postitive groin nodes
