Admission day surgery
Surgery scheduled on day of patient’s arrival at hospital
Admission orders
Written instructions by the doctor for patient care and treatment upon entry to hospital
Admission packet
Pre assembled packet of standard chart forms to be used upon admission of patient to the unit
Admission-separation form (face sheet)
Form initiated by admitting; contains personal and demographic information
Advanced directives
Documents that indicate a patient’s wishes should they become unable to make decisions regarding medical care
Allergy armband
Plastic band on which allergy information is printed, worn by patient for duration of hospital stay
Armband
Plastic band on which patient ID information is printed; worn for duration of hospital stay
cardiac arrest
When patient’s heart contractions are absent/insufficient to produce pulse
Coroner
Public officer who investigates the cause of a death not due to natural causes
Direct admission
Unscheduled admission; admitted from doctor’s office, clinic, ER
Discharge planning
Centralized, coordinated, multidisciplinary process to ensure patient has a plan for care after leaving hospital
Emergency admission
Admission necessitated due to accident or medical emergency
Isolation
The placement of a patient separated from other patients to prevent spread of infection
Kardex
Form used by nursing staff to maintain current patient profile
Medical Record Number/Unit Number
Number assigned to a patient on or before admission, used for records identification and used for all subsequent admissions to that hospital
Patient Account Number
A number assigned to the patient each time they are admitted to hospital
Pre Admit
Process of obtaining information and partially preparing admission forms before patient’s arrival at the hospital
Scheduled admission
A patient admission planned in advance (may be urgent or elective)
Stuffing charts
Placing extra chart forms in patients’ charts so they will be available when required
Supplemental chart forms
Supplemental chart forms are added to patient chart accord to specific care needs
Transcribing symbols
Notations made in red ink on the doctor’s orders sheet to indicate a step of the transcription procedure is complete
