When should you obtain vital signs?
admission, per policy or orders, change in condition, change in level of consciousness, pre- and post-op, prior to administering certain medications
what is normal temperature?
97-99 F
100.4 or greater = fever
what is a normal pulse rate?
60-100 bpm
what is a normal respiratory rate?
12-20 bpm
what is normal blood pressure?
120/80 mm Hg
what is normal O2 sat?
95-100%
what are some things that can cause your temperature to go up?
infection, heat exhaustion, sunburn, inflammatory conditions, immunizations, illicit drugs, autoimmune conditions, teething
what are some things that can make your temperature go down?
inadequate clothing, exposure to cold, wet clothing, and aging
what are some nursing interventions for increased temperature?
cool, damp cloth, fan, open windows, remove clothing/blankets, cool bath/shower, ice packs, administer antipyretics
what are some nursing interventions for decreased temperature?
apply clothing/blankets, activity, warm bath/shower, change damp clothing, warming device
what are the different routes to take temperature?
oral, tympanic, temporal, axillary, rectal
