Vital Signs Flashcards
List the 5 components to vital signs
- Temperature
- Blood Pressure
- Respiration
- Pulse
- Pain
Shell temperature
Temperature at skin surface
Core temperature
Temperature near the center of the body where organs are
4 sites for temperature measurement
- Mouth
- Rectum
- Axillary
- Ear
List 7 times when to take vitals
- On admission
- Once a day once client is stable
- Per medical order
- Every 4 hours when one or more vitals are abnormal
- Every 5-15 minutes when client is at great risk for change
- Before, during and after blood transfusion
- Before admin of medications that can affect vitals
What is the normal range of shell temperature for adults?
96.6 to 99.3 F
What is the normal range of core temperature for adults?
97.5 to 100.4 F
What are the 4 mechanisms of heat transfer?
- Radiation
- Convection
- Evaporation
- Conduction
Example of Radiation heat transfer mechanism
Uncovered head releasing heat on a cold day
Example of Convection heat transfer mechanism
Fan blowing cool air across body
Example of Evaporation heat transfer mechanism
Sweat vaporized from skin
Example of Conduction heat transfer mechanism
Application of heat or cold pack
