.This is effective as it makes clear that the speaker is determined to not be overpowered by his emotions or to express his suffering. He fears he will break down during this traumatic time and lose control therefore he is protecting himself. Initially, this may appear cold-hearted however he may be trying to keep his composure so that the ill patient, whom he cares about, does not recognise his fear and concern.

.This is effective as it suggests that the speaker is aware that he will eventually have to address the patient’s illness and the inevitability of death. He acknowledges that he is avoiding his emotions and his own suffering in order to protect himself and not break down.
