Acute vestibular loss
- At rest: Nystagmus, vertigo, oscillopsia
- With movement: Nystagmus, vertigo, oscillopsia
Unilateral vestibular hypofunction
- At rest: No nystagmus, no vertigo
- With movement: Dizziness, decreased VOR, disequilibrium
BPPV
- At rest: None
- With position change: Vertigo, nausea/vomiting, lightheadedness, nystagmus
Bilateral vestibular hypofunction
- At rest: Disequilibrium
- With movement: Disequilibrium, dizziness, oscillopsia, nausea/vomiting
Meniere’s Disease
Sudden dizziness, sometimes vomiting
Hearing loss
Vestibular Neuritis
Sudden onset; acute dizziness; symptoms = vertigo, disequilibrium, nausea/vomiting, oscillopsia
Labyrinthitis
Sudden onset; acute dizziness; symptoms = vertigo, disequilibrium, nausea/vomiting, oscillopsia; also hearing loss & tinnitus
Perilymph fistula
Conductive hyperacusis, imbalance with noise, physical effort that increases intracranial pressure (coughing); vertigo
Superior canal dehisence
- Veritgo, hearing loss, oscilopsia
- Imbalance with noise or physical effort that increases intracranial pressure (cough); tullio phenomenon (eye movements with loud noises); Hennebert sign (eye movements induced by pressure in external auditory canal); sudden or progressive hearing loss
