On examination, the anterior leg looks red, swollen and feels cooler than the rest of the limb. Dorsiflexion of the foot is impaired and so is sensation over the first and second toes. The intracompartmental pressure of his anterior compartment is 40mmHg. The pulse palpated just lateral to the extensor hallucis longus tendon is weak. dorsalis pedis artery
The dorsalis pedis artery is a continuation of the anterior tibial artery that runs on the dorsal surface of the foot. As the intracompartmental pressure of the anterior leg compartment is increased, it should compress the anterior tibial allowing only some of the blood to the dorsalis pedis. This explains its weakened pulse, which is always felt lateral to the tendon of extensor hallucis longus tendon. Together with the posterior tibial artery pulse, they are the most common pedal pulses felt.
