what are key questions to ask
how often -episodes
how long
do you feel like everything is spinning
do you feel lightheaded and that you may pass out
do you feel like your eyes are shaking/moving side to side so disorted vision
have you had headaches, nausea, vomiting
photophobia
changes in hearing
HOW LONG
red flags
head movements diplopia bowel and bladder disturbance loss of consciousness increased cranial pressure- nausea, omitting, blurred vision
what can be seen with hearing loss
Meniere disease- acute attacks lasting mins to hours and associated with hearing loss
larbriynttis -lastings hours to days associated with hearing loss too
what is not associated with hearing loss
consider time frames
seconds to mins - benign paraoysmol positional vertigo
migrane associated if mins to hours
hours to days it vestibular neuritis
what is benign parxmyosal positional vertigo
sudden movement triggered rotational vertigo
lasts seconds
no hearing loss
causes otoconia cellular debris
followred urti and head injury
classically when looking up and turning head
diagnosis - dix hallpike test - line spuine and hang head of edge of bed
turn head and side being tested is the lower ear -> positive test nystagmus
other is epleys manovre -line downa nd turn body side to side and sit up
what is vestibular neuritis
rotatory nystagmus lasting days
post URTI
rapid onset and symptoms tend to settle after afew weeks nausea and vomiting too
no hearing loss
if hearing affected ten lbarynthitis
usually iral infection of swelling of vestbular nerve
prchloperazne and cinnarazine for management

halpike test to diagnosis -hand head of the bed and turn - ear lowest to ground is tested side -may be rotatory nystagmus
