What are the common examination findings?

A

Ulcers on medial malleolus

Ulcer characteristics:

  • Painless
  • CRT normal
  • Ill defined and shallow
  • Slough
  • Varicose veins/Varicose eczema/lipodermatosclerosis/haemosiderin deposits

What are the investigations?

A

To finish the exam i would like to - ABPI, cardivascular exam

By the bed - History and obs.

Bloods - FBC, blood glucose, lipid profile

other - Doppler, swab and refer for surgery

A

Depends where the varicose veins are:

SFJ

Superficial or deep

A

Conservative - Explain what they are

Medical - Stockings

Surgical - Refer to vascular surgeon - Tie or strip

A

Primary:

  • Idiopathic
  • Congenital

Secondary:

Anything obstructing flow:

  • Obesity, valvular imcompetence, pregnancy, pelvic neoplasm
