Arterial Ulcers Flashcards
What are the common examination findings?
Ulcers on lateral malleolus or dorsal aspect of foot including top of toes
Characteristics:
- Painful
- Deep and well defined
- CRT low
- Cold and pale feet
- Low ABPI
- Hairless feet
What are the investigations?
To finish the exam i would like to - Cardiovascular exam ABPI
By the bed - History and obs.
Bloods - FBC, CRP, Lipid profile, U/E, GLUCOSE for diabetes - to see organ damage
Other - ECG for cardiovascular disease, Echo, angiography
Refer to vascular surgeon
What are the key investigation findings?
Low ABPI
Signs of arterial disease and risk factors
What is the treatment?
Conservative - Explain what they are and why they have them. Change lifestyle and be aware of clinical limb ischaemia.
MDT team to manage - Podiatry, GP etc
Medical - Reduce risk factors eg statins, DM medication, HTN medication.
Swab for infection
Surgical - Refer to vascular surgeon - Tie or strip
What are the risk factors
Smoking
Alcohol
Hypertension
Diabetes
