Cardiovascular effects of CCBs
PVR: Decrease.
Left ventricular hypertrophy: Decrease
Vasodilators used for hypertension - 1 group, 4 drugs
Calcium channel blockers. Nitroprusside. Minoxidil. Hydralazine. Fendolopam.
CCBs - indications
Angina pectoris, HT, peripheral vascular disorders, cardiac arrhythmias.
CCBs - MOA as antihypertensive drugs
Block calcium channels in vascular smooth muscle, causing vasodilation (with greater effect on arteriolar than venous smooth muscle), May have some natriuretic effect.
Diltiazem and verapamil - additional effect
Reduce HR and cardiac output.
CCBs drugs - 1 group + 2 drugs
Dihydropyridine class.

CCBs - benefits
Protect agains stroke, CHD, kidney disease.
Do not increase glucose, uric acid, lipids, electrolytes.

Asthma or African heritage
Preferred CCBs in hypertension
Amlopidine (long-acting).
SR nifedipine.
Dihydropyridine CCBs - adverse effects
Dizziness, edema, gingival hyperplasia, headache, reflex tachycardia.
Dihyrdopyridine CCBs - interactions
Azole antifungals, cimetidine and grapefruit increase serum levels
Diltiazem and verapamil- adverse effects
AV block, bradycardia, constipation, dizziness, edema, gingival hyperplasia, headache, heart failure.
Diltiazem and verapamil - interactions
Increases levels of: Carbamazepine, digoxin, theophylline
