Angiotensin inhibitors 2 (ARBs + Aliskiren) Flashcards
ARBs - cardiovascular effects
PVR: Decrease
Plasma renin activity: Increase
Left ventricular hypertrophy: Decrease
ARBs - effects on potassium
Serum potassium: Increase
ARBs + aliskiren - adverse effects
Hyperkalemia
ARBs: Neutropenia, elevated liver enzymes. Fetal injury and death
ARBs - interactions
Cimetidine - increase serum levels.
Phenobarbital - decrease serum levels
Aliskiren - effects on potassium
Serum potassium: Increase
Aliskiren - interaction
Reduced levels of furosemide.
Cyclosporine - increased levels of aliskiren
ARBs - MOA
Selectively block AT1 receptors: reduced vasoconstriction, aldosterone secretion, Na reabsoption, NR release.
ARBs - 5 drugs
Candesartan Irbesartan Losartan Talmisartan Valsartan
Losartan - benefits
Greater reduction of left ventricular hypertrophy, less risk of stroke and new onset diabetes than atenolol.
Telmisartan - benefits
Increase insulin sensitivity by activating peroxisome proliferator-activated receptor-gamma.
ARBs - contraindications
Pregnancy
Aliskiren - MOA
Binds to active site of renin and prevents formation of angiotensin I. Lowers plasma renin activity, angiotensin I and II.
Aliskiren - benefits
Protect against compensatory increases of angiotensin II caused by other drugs.
