What can go wrong with valves?
Stenosis = valve doesn't open properly during contraction = not much blood through it Incompetence = valve doesn't stop blood coming back during relaxation
What causes valve disease?
Rheumatic fever
Calcific aortic valve disease
Age related degeneration
Abnormal valves
What is rheumatic fever?
Infec in childhood that damages heart valves
What causes rheumatic fever?
Streptococcal pharyngitis
How can rheumatic fever impact the heart?
Streptococcal pharyngitis = immune rxn by body = cross rxn with self antigens = endocarditis, myocarditis, pericarditis, polyarthritis
Effects of calcific aortic valve disease?
Rigid valve cusp
Stenotic valve
LVH (left ventricular hypertension)
Inadequate coronary artery perfusion
Types of prosthetic heart valves?
Biological = human, porcine Artificial = tilting disc, ball and cage
Complications of prosthetic valves?
Haemolysis Coagulation Anticoagulation therapy Mechanical failure Calcification Infective endocarditis
What is infective endocarditis?
Bacterial infection, can occur on valve damage due to rheumatic fever
Sites of infective endocarditis?
Any abnormal heart valve

Tricuspid valve

Complications of infective endocarditis?
Local
- Valve incompetence
- Myocarditis
Systemic:
- Emboli
- General effects of systemic infection
- Glomerulonephritis

Staphylococcus aureus
Aspergillus
