When do DTs usually occur in alcohol withdrawal?
-usually in 2-4 days
When do seizures typically occur in alcohol withdrawal?
-in 12-48 hrs after last drink
Tx for alcohol withdrawal?
- benzodiazepines, esp lorazepam bc it is not metabolized by the liver!
- thiamine, for wernickes encephalopathy
- multivitamins
- folic acid, for beriberi
- haldol, for psychosis if necessary
What to do if pregnant mom refuses a tx or procedure that would prevent irreversible harm to her fetus?
-mom has a right to refuse as long as she has capacity
Tx of choice for OCD?
- SSRIs
- clomipramine (TCA) can be used if not responding to an SSRI
What is the remission phase?
- absence of all or minimal sx
- return to pt’s premorbid functioning or wellness
- pt no longer meets dx criteria for dz
What is the response phase of tx?
- pt has significant improvement
- 50% reduction in baseline severity of Ssx
What is the relapse phase of an illness?
-a return of the sx from the acute phase
What is the recurrence phase of an illness?
-when after the recovery a subsequent episode occurs
Tx of kleptomania?
-CBT is the tx of choice
Kleptomania v typical shoplifting?
- NOT premeditated
- anxiety before stealing that is relieved by stealing
- feel guilty or remorseful after stealing
What are the 5 parts of the Hospice model?
- Focus on quality of life, not cure or prolongation
- Symptom control (pain, nausea, anxiety, etc)
- Interdisciplinary team
- Services provided at home or living facility
- Requires survival prognosis of < or = 6 mnths
