What is a lower UTI?
Infection of bladder - cystitis
What is an upper UTI?
Infection up to kidneys - pylonephritis
What do UTIs increase the risk of during pregnancy?
Preterm delivery
What is asymptomatic bacteruria?
Presence of bacteria in urine without symptoms of infection
When are women tested for asymptomatic bacteriuia?
At booking nad routinely throughout pregnancy, tested using urine sample sent for MC&S
Most common organism cause of UTI?
E-coli
Symptoms suggestive of lower UTI?
- Dysuria
- suprapubic pain
- increased frequency
- urgency
- incontinence
- haematuria
Symptoms suggestive of pyelonephritis?
- fever
- loin / back pain
- vomiting
- loss of appetite
- haematuria
- renal angle tenderness
What can be seen on urine dipstick for UTI?
Nitrites : gram-ve bacteria (e-coli) more accurate for infection than leukocytes
Leukocytes : WBCs suggesting infection
What is more accurate indication for infection nitrates or leukocytes on urine dipstick?
Nitrates
How to manage UTI OR asymptomatic bacteriuria? Pregnancy specific
7 days of abx:
-> Nitrofurantoin *
- avoid close to term due to neonatal haemolysis risk, use amoxicillin or cephalexin
What to use if mother has bacteriuria close to term?
nitrofurantoin to be AVOIDED
Use amoxicillin or cephalexin
What is the UTI is due to GBS (group B strep)?
Managed with IV benzylpenicillin
