A 29-year-old woman presents with a 5-day history of low mood after giving birth to her first son a week ago. She feels more irritable and anxious than usual, occasionally bursts into tears, and is worried about being unfit to care for her baby.
Baby blues
A new mother presents with poor sleep, getting about 4 hours a night since her baby was born 3 weeks ago. She becomes tearful, feeling overwhelmed and inadequate as a mother. She believes others think she’s a bad mother and that her mother-in-law is trying to take her baby.
Postpartum psychosis
A 25-year-old para 1+0 presents at 36 weeks with painless vaginal bleeding. She reports that she has had intermittent spotting over the last 4 weeks, but they have increased in volume and frequency. Her blood pressure is 125/80mmHg and her heart rate is 85bpm. On examination, her abdomen is soft and non-tender, and the fetal head is not engaged and high.
