uncomplicated cystitis first line
nitrofurantoin 100 mg PO bid x 5d, TMP/Sulfa 800/160 mg PO BID x 3d, fosfomycin 3 g PO x 1 dose
uncomplicated cystitis alternate treatments
cipro 500 mg PO BID x 3d, levo 250 mg PO daily x 3d, augmentin 500 mg PO TID x 5-7d, cefdinir/cefpodoxime x 3-7d
complicated cystitis treatment
cipro 500 mg PO BID x 7-10d, levo 250 mg PO daily x 10d (or 750 x 5d), sulfa/tmp 800/160 mg PO BID x 7-10d
complicated cystitis treatment for pregnancy
augmentin 500 mg PO TID x 7-10d
uncomplicated pyelonephritis treatment
cipro 500 mg PO BID x 7d, levo 250 mg po daily x 7d, TMP sulfa DS PO BID x 14d
uncomplicated pyelonephritis treatment if gram + cocci
augmentin 500 mg PO TID x 14d following ceftriaxone IV
