What is Epiglottitis?
Inflamm of Epiglottis + nearby supraglottic structures
Why can Epiglottitis be life threatening?
Can cause airway obst
What are the Risk Factors of Epiglottitis? (6 things)
- Children (6-12 yrs)
- Unimmunized status
- Mucosal trauma (e.g burns / foreign body ingestion)
- DM
- Substance Abuse
- BMI under 25
What are the most common causes of Epiglottitis in Kids vs Adults?
- H influenzae (kids)
- Strep pneumoniae (adults)
(Both bacteria)
What are the CF of Epiglottitis in kids? (4 things)
Hallmark CF = 4 D’s
- Dysphagia
- Dysphonia
- Drooling
- Distress
RAPID progress
What are the CF of Epiglottitis? (11 things)
- Severe sore throat (most common complaint in adults)
- Dysphagia + odynophagia (difficulty + pain)
- Dysphonia
- Drooling
- Distress
- Fever
- Stridor
- Tachypnoea
- Cyanosis (bc difficulty breathing –> resp distress)
- Anxiety + refuses to lie down
- Assumes tripod position (see pic) –> to maximise diameter of narrowed airway
What is the difference in PROGRESSION of Epiglottitis in kids vs adults?
- Kids = RAPID progression
- Adults: slower progression
If sus Epiglottitis, what should you do before further investigations?
Airway management bc med emergency
What investigations should you do for sus Epiglottitis? (3 things)
- Laryngoscopy (done under general) (GOLD)
- Throat culture (positive)
- XR
- Bloods
What will be seen / felt in a Laryngoscopy of Epiglottitis? (2 things)
- Inflamm Epiglottis (+ supraglottic structures)
- Hyoid bone tenderness
What will be seen in an XR of Epiglottitis?
“Thumb sign”: shadow of enlarged epiglottis (confirms diagnosis)
What will the bloods show in Epiglottitis? (2 things)
- Raised WBC
- Raised CRP
What are the management options for Epiglottitis? (2 things)
- Airway management (w humidified O2)
- Abx: 3rd gen cephalosporin (if H influenzae)
What are the complications of Epiglottitis? (4 things)
- Airway obst
- Oropharyngeal secretion aspiration
- Cardiopulmonary arrest
- Death
