Testicular torsion
Young adolescent with very acute severe testicular pain
No fever, pyruia, mumps
Testis is swollen and hi riding with horizontal lie
Must surgically fix within 4 hours
Cord is not tender
US to determine blood flow
Acute epididymitis
Young men who are sexually active
Fever and pyruia
Swollen and tender testis
Normal position
Cord is tender
US to rule out torsion
Acute bacterial prostatitis
Older men
Chills, fever, dysuria, urinary frequency, low back pain, tender prostate on rectal exam
IV abx
Don’t keep doing prostatic massage because –> septic shock
Posterior urethral valves
Newborn boy not to urinate during 1st day of life
VCUG for diagnosis
Hypospadias
Pee on your shoes
Don’t circumcise until you’ve fixed it
UTI in child
Urologic workup because they might have reflux
Hematuria workup
CT scan
Cystoscopy
Renal cell carcinoma
Hematuria, flank pain, flank mass
Hypercalcemia
Erythrocytosis
Elevated liver enzymes
Heterogenic solid tumor
Transitional cell carcinoma
SMOKERS!!
Hematuria painless
CT scan
Prostate cancer
PSA
Transrectal needle biopsy for diagnosis
CT scan for therapy choice - radiation and/or surgery
Testicular cancer
Young men with a painless testicular mass
Biopsy is a radical orchiectomy via inguinal route
alpha feto protein and BHCG
Acute urinary retention
Precipitated during a cold, antihistamines, abundant fluids, etc… in males
Leave indwelling catheter in for 3 days
Long term - tamsulosin (alpha blocker), finasteride (5-alpha reductase)
TURP
Pneumaturia
Fistula between bladder and GI tract (sigmoid colon) from DIVERTICULITIS
CT scan then surgery
Organ transplant hyperacute rejection
Vascular thrombosis within minutes
Preformed antibodies (mismatched RBC) - prevented by ABO blood matching
Organ transplant acute rejection
Within first 3 months while patient is on immunosuppressives (tacrolimus, mycophenolate mofetil +/- prednisone)
