Bacteriuria
any growth on catheter w/ sx; > 10E3 CFU w/ sx or catheter; > 10E5 CFU in asx
Positive urinalysis
Bacteriuria, leukocyte esterase (+), nitrate (+), pyuria
Complicated UTI Definition
infection in males or pt w/ structural GU abnorms
ASx Bacteriuria When to Treat
prego, urologic surg, neutropenic, renal transplant
Lower UTI (cystitis) Sx
dysuria, frequency, urgency, suprapubic tenderness
Upper UTI (pyelonephritis) Sx
flank PA, fever, n/v; severe: dehydration, s/s of sepsis
Catheter-assoc UTI Sx
fatigue, chills, confusion, suprapubic tenderness
UTI Sx in Elderly
AMS, profound weakness, frequent falls, change in eating, GI sx
Pyuria
WBC > 10/mm3
UTI Pathogens
Enterobacter, E. coli, Kleb, Proteus, Pseudo aeruginosa, S. saprophyticus
Uncomplicated UTI Tx
Bactrim (susp > 80%), cephs (cephalexin, cefpodoxime, cefdinir), nitrofurantoin, FQs (2nd line); for 3-5 days
Complicated UTI Tx
Bactrim (susp > 80%), cephs (cephalexin, cefpodoxime, cefdinir, cefazolin;), nitrofurantoin, FQs (2nd line) for 7-14 days
Pyelonephritis UTI Tx
IV: ceftriaxone, PO: FQ, Bactrim, Cefpodoxime; for 7-14 days
CA-UTI Tx
ertapenem, ceftriaxone, FQs; for 7-14 days
Severe UTI Tx
cefepime; PCN all: aztreonam +/- gent
