Neutropenic fever definition
A
Neutropenia: ANC (or expected) less than 500
Fever: greater than 38.8C or 38C x 2

High risk neutropenia
A
Anticipated prolonged profound neutropenia following chemo and/or significant comorbid condition; start empiric tx

High risk empiric abx
A
antipseudomonal BL (cefepime, ceftazidime, zosyn, then carbapenem) +/- vanco

Empiric antifungal in neutropenia
A
Fever despite 4-7 days of empiric abx, micafungin or voriconazole then AmBisome

Low risk/step-down abx
A
cipro + augmentin; moxifloxacin

PCN all empiric abx
A
aztreonam + gent + vanco

Neutropenic tx duration
A
appropriate antimicrobial tx should cont for at least the duration of the neutropenia
