what is chronic urinary retention
gradual development over months = inability to completely empty bladder
what causes benign prostatic hyperplasia in men
enlarged prostate
what are the symptoms of benign prostatic hyperplasia
- urinary retention
- urinary urgency
- urinary frequency
- urinary nocturia

- immediate catherisation due to pain
- alpha blocker for 2 days before removing catheter

what does long-term use of catheter cause
- recurrent UTI
- urethra trauma
- pain
- stone formation
benign prostatic hyperplasia urinary retention tx
alpha blockers to relaxe smooth muscle
urinary retention tx if enlarged prostate, raised prostate antigens or high risk of progession

5 alpha reducatase inhibitor = finasteride or dutasteride

- defuzosin
- doxazosin
- tamsulosin
- terazosin
when to avoid alpha blockers
- micturition synacope
- postural hyoptension
micturition synacope = fainting during or immediately after urinating
s/e of alpha blockers
- dizziness
- postural hypotension
what happens with 1st dose of alpha blocker and when to take
- 1st dose = collapse due to hypotensive effect
- take before bed
- lie down = dizziness, fatigue or sweating develop
5 alpha reducatase inhibitor s/e
finasteride or dutasteride
- breast disorder
- sexual dsyfunction
- male breast cancer reported
- finasteride = reports of depression and rare cases of suicidal thoughts (STOP+REPORT)
5 alpha reducatase inhibitor caution
- excreted in semen = condom if pregn partner
- women = child-bearing age avoid crushed broken tabs
symptoms to report if male cancer is suspected in 5a reductase inhibitors
- lumps
- pain
- nipple discharge
