Assessment Data to Include
Identify urinary alterations, Pattern of urination, skin and mucous membranes, kidneys, bladder, characteristics of urine, urine testing, intake and output.
Factors influencing urination
Age, environmental factors, medication history, psychological, muscle tone, fluid balance( nocturnal enuresis, urinary frequency, and muscle tone.
Implications for nursing diagnostic tests
Obtain a signed consent, Assess for allergies, administer bowel cleaning medications, ensure pt receives proper pre-test instructions, such as NPO or clear liquid. Assess I&O, Observe urine, encourage fluid intake,
Measures to reduce incontinence
Stimulating micturition reflex, have a routine elimination habit, adequate fluid intake, promote complete bladder emptying, prevent infections.
Reducing UTI
Sufficient fluid intake, 1200-1500 ml day, voiding after intercourse, not using lots of soap or taking bubble baths, wear cotton underwear, apple or cranberry juice.
