Urinary incontinence is defined as
Involuntary leakage from the bladder during the storage phase of micturition
Congenital causes of urinary incontinance
- ectopic ureters
- USMI
- bladder hypoplasia
- patent urachus
Acquired causes of urinary incontinance
- USMI
- Overflow incontinence - excessively large flaccid or tense bladder.
- UTI
- detrusor hyperexcitability
- PU/PD - excessive challange
Best way to diagnose ectopic ureters
Cystoscopy
Only positive test for USMI
Urodynamics - urethral pressure profile - see a flatline where you would normally see muscle contractions.
Overflow incontinance can be caused by
Neurological conditions, bladder atony, urethral/bladder neck obstruction
Acquired USMI
Usually post neutering - alteration in hormonal influences & collagen:muscle ratio in urethral wall.
Tx options intramural ectopic ureters
- cystic laser ablation
- side-side neoureterostomy
- urethral re-implantation
- ureteronecphrectomy (salvage)
Tx options for extramural ectopic ureters
Urethral re-implantation
Ureteronephrectomy
USMI medical treatment
- Phenylpropanolamine
2. oestriol (neutered only)
USMI surgical options
- relocation of bladder neck - when dog lies down pressure is on both bladder & neck - reduced urine leakage.
- colposuspension more common option - permanent sutures between vaginal wall and prepubic tendon. - artificial urethral spincter - 6wks recovery then cuff inflated - incremental increase if incontinence persists.
