§ 208.41 SMALL CLAIMS PROCEDURE
A small claims action shall be instituted by a plaintiff or someone on his behalf paying the filing fee ($______ for claims of $1,000 or less; and $______ for claims above $1,000), and by supplying to the clerk the following information:
