What muscles are tested in prone?
Rhomboids
Middle Trap
Lower Trap
What side is the patient facing, and what side is the PT on for the prone muscles?
Face is towards patient while patient is on opposite side of limb being tested
Lower Trapezius testing
Shoulder abducted 130 degrees on opposite limb. thumb towards celing. Face away from side being tested.
Hand stabilizes the closest scapula/trap
Other hand palpates lower trap as hand goes up and promptly goes in presses into deltoid divit
Middle trap testing
Prone, face towards patient. Patient near other side of table with shoulder externally rotated and perpendicular to table. Thumb towards ceiling if possible.
Hold trap down and palpate medial of medial border of scapula.
Arms goes up, and palpation hand goes to scapula/shoulder divit.
Rhomboids testing
Hand on lower back that is being tested. PT opposide side of patient. Face to PT. Hand on medial border of scapula for palpation. Hand then is pressed into scapula of back.
Trapezius OIAN:
O: External occital protuberance, medial 1/3 superior nuchal line. Spinous processes of C7-T12
I: lateral 1/3 clavicle , acromion, spine of scapula,
A: elevates, retracts, rotates scapula
middle fibers retract
lower fibers depress
N: spinal accessory CN 11 and c3,c4
Rhomboids OIAN:
O: Major --> SP t2-t5 Minor --> c7-t1 SP I: medial border of scapula A: retracts and inferiorly rotates lateral angle of scap N: dorsal scap c4,c5
Lats OIAN
O: SP of t6-t12 , thoracolumbar fascia, iliac crest and lower 3-4 ribs
I: inferior of intertubecular groove of humerus
A: extends humerus, adducts humerus, medially rotates
N: Thoracodorsal C6,C7,C8
Teres Major OIAN
O: Inferior angle of scapula
I: Medial lip of intertubucular groove of humerus
A: adducts and medially rotates arm
N: lower subscap c5-c6-c7
Latissimus Dorsi/Teres Major testing
Opposide side. Hold down scapula. Ask patient to internally rotate opposite arm and hyperextend arm. Palpate teres major. Move arm to above elbow and press down and away!!!
Testing Posterior Deltoid
Same side. face away. bring elbow in L shape. Hold down opposite scapula, palpate rear delt and place that hand on elbow.
Testing shoulder IR
Prone, face away. Same side. Cloth under elbow. Bring palm inside facing ceiling
Testing shoulder ER
Prone, face away same side. Cloth under elbow. Bring back of hand towards ceiling
Which muscles IR shoulder?
Teres Major, Lats, Pec Minor, Subscap
Which muscles ER shoulder?
Teres minor, infraspinatus
OIAN Teres minor:
O: Superior part of lateral border of scapula
I: Inferior facet of greater tubercle of humerus.
A: externally rotate shoulder
N: axillary c5.c6
OIAN Infraspinatus:
O: infraspinous fossa
I: middle facet of greater tubercle of humerus
A: laterally rotate arm
N: suprascapular c5, c6
Pectoralis Major Clavicluar fibers Test
Have patient supine, bring arm overhead bent at 90 over face. Have patient go through full ROM then set at 90 degrees. hold opposite shoulder down and pull on bicep
Pectoralis Major Sternal fibers test
Have patient supine bring arm around 120 degrees and go through full rom into opposite hip. Hold opposite hip down and pull arm
Test triceps
Supine elbow at 90 degrees over head. Palpate as elbow is extended. Hold shoulder and press down on wrist.
What muscle is performed both bilaterally and unilaterally?
Levator Scapulae and Upper trapezius
Bilateral Lev Scap and Upper Trap
Shoulder shrug
Unilateral Lev Scap and Upper trap.
Instruct patient to bring ear to adjacent shoulder and rotate head slightly to the opposite side.
Ask patient to raise shoulder to that ear and and press more on shoulder than head.
Serratus Anterior:
Sitting. Arm straight out punching out as you will.
Arm above elbow and other hand pushing in and adducting scapula. In and around.
