Upper Extremity Nerves Flashcards
What dermatomes does the axillary nerve innervate?
C5-C6
What types of injuries damage the axillary nerve?
- Fractured surgical neck of the humerus
2. Anterior dislocation of the humerus
What is the presentation of an axillary nerve injury?
- flattened deltoid
- loss of arm abduction at shoulder (> 15 degrees)
- loss of sensation over the deltoid muscle and lateral arm.
What dermatomes does the musculocutaneous nerve innervate?
C5-C7
What types of injuries damage the musculocutaneous nerve?
- midshaft fracture of the humerus
- Compression of the axilla (e.g. due to crutches or sleeping with am over chair) –> commonly referred to as “Saturday night palsy”
What is the presentation of a musculocuatenous nerve injury?
- Wrist drop: loss of elbow, wrist, and finger extension
- Decrease grip strength (wrist extension is necessary for maximal action of the flexors)
- Loss of sensation over the posterior arm/forearm and dorsal hand.
What dermatomes does the median nerve innervate?
C5-T1
What types of injuries damage the median nerve?
- supracondylar fracture of the humerus (proximal lesion)
- carpal tunnel syndrome
- Wrist laceration (distal lesion)
What is the presentation of a median nerve injury?
- “Ape Hand”
- “Pope’s Blessing”
- Loss of wrist flexion, flexion of lateral fingers, thumb opposition, lumbricals of 2nd and 3rd digits
- Loss of sensation over the thenar eminenceand dorsal and palmar aspects of the lateral 3 1/2 fingers with proximal lesions
- Tinel sign: tingling on percussion of median nerve seen in carpal tunnel syndrome.
What dermatomes does the ulnar nerve innervate?
C8-T1
What types of injuries damage the ulnar nerve?
- Fracture of the medial epicondyle of the humerus (“funny bone”), proximal lesion
- Fractured hook of the hamate, distal lesion
What is the presentation of an ulnar nerve injury?
- “Ulnar Claw” on digit extension
- Radial deviation of the wrist upon flexion, proximal lesion
- Loss of wrist flexion, flexion of the medial fingers, abduction and adduction of fingers (interossei), actions of medial 2 lumbrical muscles
- Loss of sensation over medial 1 1/2 of fingers including the hypothenar eminence.
What dermatomes does the recurrent branch of the median nerve innervate?
C5-T1
What types of injuries damage the recurrent branch of the median nerve?
- Superficial laceration of the palm
What is the presentation of a recurrent branch of the median nerve injury?
- “Ape hand”
- Loss of the thenar muscle group: opposition, abduction, and flexion of the thumb.
- No loss of sensation is present.
