UE MSK Lab/DSA Flashcards
humerus BLT
stand on affected side, patients arm holding opposite clavicle
hands around humerus, thumbs over head
leverage with humerus to chest and pull laterally
patient moves uninvolved shoulder posteriorly
gently int/ext rotate humerus and superior until feel change in tissue tension
weight of force used for BLT
no more than 5 pounds
position for scapulothoracic joint BLT
thumb under scapula with pad against ribs nail against scapula
patient leans back so can contact serratus anterior
other hand on scapula contact to spine and disengage inf medial scapula border
Tx Post radial
pronation
axial force bring into supination
closed reduction nursemaids elbow
palpat radial head
axial compression at wrist while supinating and flexing elbow
closed reduction nursemaids elbow
palpate radial head
axial compression at wrist while supinating and flexing elbow
girls or boys more affected by nursemaids
girls
mm to balance in scapulothoracic BLT
serratus ant
rhomboids
teres
hypertonic serratus anterior does what to scapula
elvation and lateral displacement
who is fulcrum who is activating force in BLT
patient activating force
physician fulcrum
motion mechanics between bones and joint
change in shape of joint space
motion mechanics between bones and joint
change in shape of joint space
Chin pivot used for what SD
T1-T3
Tx FRSL T1 chin pivot
chin on table
lift patients head and neck sidebend until motion palpated, then head back on table
heel of L hand post transverse proecss T1 and lateral caudal force
R hand on L side rotate head to L to localize
sidebend and rotation forces in chin pivot HVLA
sidebend toward restriction and rotate to ease
final force in chin pivot T1FSRL
anterior lateral caudal from L hand to L transverse T1
indications b/l shoulder balancing
SD sternoclavicular, acromioclavicular and glenohumeral
Tx b/l internally rotated humerus with b/l balancing
pals hands on AC joints with fingers over proximal humeruses
both hands asses shoulder motion all planes and find equal tension in all hold until release
tenderpoints shoulder for counterstrain
bicepital groove- biceps
ant, inf lateral scapula- subscapularis
above scapular spine- suprascap
forearm and wrist tenderpoints
medial epincondyle
radial head
palmar wrist and dorsal wrist
position for supraspinatus counterstrain
flexion ABduction to 45 degrees and ext rotation
position for subscap counterstrain
extention
slight ABduction
internal rotation
position for counterstrain biceps brachii
flexion elbow, minor flexion arm AD duction and internal rotation
mm for biceps brachii counterstrain
brachialis and biceps brachii
position for counterstrain radial hear
full extension elbow
supination of wrist and fine tune with AB/ADduction
position counterstrain medial epicondyle and mm involved
flex elbow 90 degrees and pronate wrist, fine tune with int/ext rotation
forearm flexors and pronator teres
position counterstrain dorsal wrist
wrist extension and fine tune with radial ulnar deviation
use wrist extensors
position counterstrain palmar wrist
wrist in flexion, fine tune with radial ulnar deviation
use wrist flexors
position counterstrain palmar wrist
wrist in flexion, fine tune with radial ulnar deviation
use wrist flexors
Stages of Spencers
